Service Agreement

The Service Agreement should be completed for each confirmed customer and include the sections described below.
Customer and Service Information
- Customer's full name.
- Telephone number and email.
- Current address.
- Destination address.
- Scheduled date and arrival window.
- Services selected.
- Inventory or estimated scope.
- Property-access details.
- Specialty items.
- Transportation arrangements.
Scope of Services
The agreement should identify which services are included:
- Consultation and planning.
- Packing.
- Packing materials.
- Loading.
- Unloading.
- Move-day management.
- Transportation.
- Senior-transition support.
- Business relocation.
- Unpacking.
- Furniture assembly or placement.
- Donation, disposal, or storage coordination.
Services not listed in the signed agreement are not included unless added in writing.
Pricing and Deposit
The agreement should show:
- Estimated service price.
- Required $150 deposit.
- Confirmation that the deposit is nonrefundable.
- Remaining payment schedule.
- Hourly or project rates.
- Minimum service hours.
- Material charges.
- Transportation or rental costs.
- Additional-service rates.
- The $75 cancellation fee.
- Late-payment terms.
Changes to the Scope
Additional items, labor, stops, time, stairs, packing, waiting, specialty handling, or access challenges may result in additional charges.
Changes should be documented and approved by the customer or authorized representative.
Customer Authorization
The customer authorizes Stephens Relocation & Transition Management to perform the services described in the agreement and confirms that the customer has authority to authorize access and handling of the listed property.
Cancellation and Rescheduling
The agreement should repeat:
The $150 deposit is nonrefundable. Cancellations and rescheduling requests must be submitted at least 48 hours before the scheduled service. Every cancellation is subject to a flat $75 cancellation fee, which is separate from the forfeited deposit, plus any nonrecoverable expenses stated in this agreement.
Signatures
Include:
- Customer signature.
- Customer printed name.
- Date.
- Company representative signature.
- Date.
- Optional electronic-consent acknowledgment.
Legal Review
Because Georgia separately regulates in-state movers and interstate moves fall under federal oversight, the final Service Agreement should be reviewed by a Georgia attorney familiar with transportation and household-goods services before paid transportation begins.
